Important Refill Request Information


Before submitting your medication refill request, please review the following important

reminders:


We require at least 72 business hours' advance notice for all medication refills. This allows time to address any delays or issues that may arise and helps ensure that you do not run out of your medication.


A follow-up appointment with your provider must be scheduled. If you do not have one scheduled, please call our office at (480)-546-2819 to set it up before submitting your request.


Refill requests will not be processed for accounts with an overdue balance. You must pay your balance in full before a refill can be approved. If you have spoken to our office about your balance, you may skip this reminder. If you need assistance with your balance, please contact our office at the number above.


Before submitting your request: 


Please check with your pharmacy to confirm that you do not already have remaining refills on your most recent prescription.


Our administrative team handles refill requests during business hours only:

Monday through Thursday, 9:00 AM – 5:00 PM and Friday 9:00 AM - 12:00 PM. Requests submitted on weekends or holidays will be reviewed the next business day.


Do not call the office to check the status of your request, we will reach out as soon as your request has been reviewed.
